Schematic ahd actual circuit show fan, for example, as being controlled by pin 44. In pins.h for the 1284p, pin 4 is specified.
Similarly, the heatbed control pin is specified as 10 or 12, but is actually wired to 13 (or 14; the one next to the pin for the extruder heater).
THus, no fan operation, and no heatbed operation. Extruder and thermistor assignments do work (well, reversing thermistor inputs).

I verified continuity on the board as 44, 13, 14 etc., so that schematic seems to be correct.
Using the 1284p melzi schematic pinout for the flatpack 44 pin SM device (on my board) I tried to change the pins.h values to the defs
shown. I get back an erorr message during compile.
Have not tried testing axis motion yhet, but the extruder stepper does respond, so at least those pins are correct - or close.
